Causes and Risk Factors for the Development of Hemorrhoids

The development of symptomatic hemorrhoids has been associated with advancing age, diarrhea, pregnancy, pelvic tumors, prolonged sitting, straining, chronic constipation, and patients on anticoagulation therapy. The actual process by which hemorrhoids develop is not well understood. Most hemorrhoids, however, are the result of constipation, straining and sitting on the toilet for long periods of time.

Causes of Hemorrhoidal Disease

Ulitmately, what causes any form of Hemorrhoidal Disease is extended periods increased pressure in the anal cushions. Although, there are many conditions which can lead to this increased pressure, any amount or type of straining can lead to an inflamed hemorrhoid. Some of the major contributors to hemorrhoids are:

  • Strenuous or extended physical activity
  • Heavy lifting, including working out / weightlifting
  • Pregnancy / labor
  • Frequent constipation or diarrhea
  • Low-fiber diets
  • Poor bowel habits
  • Extended periods of sitting
  • Obesity
  • Genetics
  • Aging

Prevention is often a matter of carefully regulating physical activity and practicing sound habits regarding nutrition and bowel habits. Take frequent breaks between short sessions of exercise, eat fiber-rich foods or take a supplement, and avoid waiting long periods of time after you get the urge to go to the bathroom. Dr. Adam Gropper is a board-certified Diagnostic & Interventional Radiologist. Before undertaking his medical training, Dr. Gropper was an engineer at General Dynamics Corporation, working on various projects including cruise missile guidance systems, “Star Wars” strategic defense simulations, and others. He is a lifelong “techie,” and has always been an early adopter and innovator of new technologies in treating vascular problems with minimally invasive approaches.

Dr. Gropper received his M.D. from Emory University, where he also completed his residency and fellowship training in Vascular & Interventional Radiology. He has been practicing in Florida since 1999. He has served as an Associate Professor at FIU Medical School and recently retired from his position as Chief of Radiology at Jackson North Medical Center after 10 years of service.

It has been Dr. Gropper’s lifelong dream to have his own office-based practice where he could spend plenty of time taking care of his patients in a high tech, patient-centered atmosphere of his own design. He created Vivid Vascular to do exactly that. With his new state-of-the-art cath lab, he has been able to provide complete care by being able to perform procedures in his areas of expertise in the office, ensuring the best and most comfortable care for each patient. He has performed thousands of vascular procedures on the arterial and venous systems, and has many years of experience in uterine fibroid embolization and pelvic pain issues, hemorrhoid & prostate embolization, interventional oncology, biopsies, spine interventions and dialysis interventions.

“It’s been so rewarding to build this state-of-the-art practice centered around my patients’ needs,” said Dr. Gropper. “I love seeing my patients walk out the door happy and healthier.”

Dr. Gropper and his wife Kari live in Miami Shores. They have three children and spend their leisure time boating, fishing, snow skiing and traveling.

How are Hemorrhoids treated

For patients who do not respond to conservative treatments such as creams, soaking and laxatives/fiber supplementation, there are several options for treatment. First, in office treatments are used, most commonly rubber band ligation (RBL). Other treatments can involve sclerotherapy or infrared coagulation. These procedures involve direct manipulation of the hemorrhoids through the anus with an endoscope. Some patients, for instance, patients taking anticoagulation, may not be good candidates for these procedures. In addition, 20% of patients end up needing more invasive surgery, which requires long recovery and painful postoperative courses. These procedures can involve long and uncomfortable post-procedure recovery periods.

One of the newest treatment options is embolization of the superior rectal artery and is often a good alternative to invasive surgery, or in patients who are not candidates for other procedures.

Medications

Pain relievers, including acetaminophen, ibuprofen, and aspirin, may help with your hemorrhoid symptoms. You can choose from a variety of over-the-counter creams, ointments, suppositories, stool softeners and medicated pads. They contain medicines like lidocaine to numb the area, or hydrocortisone or witch hazel, to reduce swelling and itching.

Hemorrhoid Banding

Hemorrhoid banding, or hemorrhoid ligation, is a procedure to remove hemorrhoids by putting tight bands around the swollen veins in the lower rectum and anus. The procedure, also known as the CRH O’Regan System, uses disposable bands that cut off the blood supply to hemorrhoids.

Surgery

Hemorrhoidectomy is appropriate when you have: very large hemorrhoids, internal hemorrhoids that still cause symptoms after nonsurgical treatment, large external hemorrhoids that cause significant discomfort and make it difficult to keep the anal area clean, both internal and external hemorrhoids, or if you’ve had other treatments for hemorrhoids (such as rubber band ligation) that have failed.

Hemorrhoid Embolization

Hemorrhoid embolization is an interventional radiology treatment for internal hemorrhoids that offers long-lasting relief from rectal bleeding and pain. Hemorrhoids form when abnormal connections are made between arteries and veins in the rectum. Embolization works by blocking these abnormal blood vessel connections, which eliminates the bleeding and pain. The main advantages to embolization is that it is  very low risk and has virtually no recovery time or recovery period discomfort, no special post-procedure care or soreness and rapid (within a few days) relief of symptoms.

What is Superior Rectal Artery Embolization (Hemorrhoid Embolization), and how does it work?

SRA embolization is a minimally invasive procedure that uses a tiny catheter inserted into the femoral artery at the top of the leg which is guided into the arteries supplying the hemorrhoids. Tiny particles are injected through the catheter, which blocks the arteries, causing the hemorrhoids to stop bleeding and shrink.

The advantage of SRAE is that it is easily tolerated and performed as an in-office procedure with minimal sedation. Patients experience little to no discomfort and go home a few hours after the procedure. Multiple studies have shown improvement in patient symptoms in over 75% of patients with no major or minor complications, comparing favorably to 7-15% rates of complications with surgical procedures.
